Here – in an easy-to-print format – are many of the forms
you need to administer NSHEPP.
Once a form has been completed and signed, please return it to us by mail. Don't forget
to make copies for you and, if relevant, the member.
Forms marked with an asterisk (*) are "interactive" forms. This means they can be filled out
on screen (although, if you wish, you can still print them out and complete them by hand). Please note that if you fill out
a form on screen, you must still print out the completed form, sign it, and return it by mail. You cannot submit the form
Because many of the forms require information that can only
be provided by the employer, members must complete forms in conjunction
with their employer. Likewise, forms should be submitted by the
employer – not the member.
